Factores de riesgo asociados a multirresistencia bacteriana en el Hospital Nacional Daniel Alcides Carrión 2018 – 2020
Abstract
Objetivo: Determinar los factores de riesgo asociados a infección por bacterias
multirresistentes en el Hospital Nacional Daniel Alcides Carrión en los años 2018
- 2020.
Materiales y métodos: Se realizó un estudio observacional, analítico, tipo casos
y controles; se usaron 180 historias clínicas de los pacientes hospitalizados con
infección bacteriana, 90 con infección por bacterias multirresistentes fueron los
casos y 90 con infección por bacterias no multirresistentes fueron los controles.
Resultados: Como resultado se encontraron asociaciones significativas para el
sexo masculino OR: 4.29 IC 95% (2.19-8.39), la edad OR: 1.02 IC 95% (1.01-
1.04), las áreas de cuidados críticos OR: 54.22 IC 95% (6.72-437.32), las áreas
de hospitalización OR 6.77 IC 95% (3.38-13.57), Diabetes Mellitus OR: 2.06 IC
95% (1.06-4.00), Enfermedad Renal Crónica OR: 5.84 IC 95% (2.11-16.16),
secuela neurológica OR: 4.53 IC 95% (1.74-11.80), Cáncer OR: 4.90 IC 95%
(1.34-17.83), hospitalización previa OR: 4.56 IC 95% (1.86-11.20), cirugía previa
OR: 4.86 IC 95% (1.73-13.6), uso previo de cefalosporinas OR: 12.57 IC 95%
(2.84-55.62), uso previo de carbapenémicos OR: 8.68 IC 95% (1.06-70.93), uso
previo de fluroquinolonas OR: 8.80 IC 95% (1.95-39.72), uso de sonda vesical
OR: 10.72 IC 95% (4.65-24.71), uso de catéter venoso central OR: 25.43 IC 95%
(3.33-194.12) y nutrición enteral OR: 5.38 IC 95% (2.09-13.90). Para el modelo
multivariado fueron importantes tener Enfermedad Renal Crónica OR: 5.92 IC
95% (1.60-21.90), la hospitalización previa OR: 3.61 IC 95% (1.18-11.10) y la
terapia antibiótica previa al evento OR 4.17 IC 95% (1.11-15.58).
Conclusión: La Enfermedad Renal Crónica, la hospitalización previa y la terapia
antibiótica previa al evento son factores de riesgo para adquirir una infección por
bacterias multirresistentes.
