Impacto de la amplitud de distribución Eritrocitaria-RDW preoperatoria como factor pronóstico en la sobrevida de pacientes intervenidos quirúrgicamente por cáncer gástrico en el Hospital Nacional Alberto Sabogal Sologuren, 2013 – 2014
Abstract
Objetivo: Determinar la relación entre la Amplitud de Distribución Eritrocitaria
preoperatoria y la sobrevida en pacientes con Cáncer Gástrico en el Hospital
Nacional Alberto Sabogal Sologuren durante los años 2013-2014
Materiales y métodos: Estudio de tipo observacional, longitudinal, analítico y
retrospectivo. Se obtuvieron los datos de las Historias Clínicas, se incluyeron
pacientes en Estadio Clínico I y II. Se realizó la estimación de Kaplan-Meier con
seguimiento a 5 años y la Regresión de Cox.
Resultados: Se incluyeron 156 pacientes. La edad promedio fue de 66.5 ±13
años. El 75.3% de los pacientes fallecidos fueron de sexo masculino. El
Estadío Clínico tipo II fue mayoritario con un 79.4%. El tipo histopatológico
Difuso fue el más frecuente con un 76.92%. La media del RDW fue de 14.68 ±
1.8%. En el análisis bivariado el RDW elevado mostró pobre supervivencia a 5
años (p=0,0001, HR=2,08, IC 95%: 1.4 - 3.1). En el análisis multivariado se
encontró relación entre el RDW elevado, el Estadío Clínico II y el no empleo de
la Quimioterapia Adyuvante con una pobre supervivencia a 5 años (p=0,0001,
HR=2,19, IC 95%: 1.43 - 3.33); (p=0,037, HR=0.6, IC 95%: 0.37 – 0.97);
(p=0,040, HR=0.64, IC 95%: 0.42 – 0.98) respectivamente.
Conclusiones: La Amplitud de Distribución Eritrocitaria (RDW) elevada,
demostró ser un factor predictivo de baja supervivencia a 5 años. El Estadío
Clínico II y no recibir quimioterapia son predictores de mortalidad.
