Factores del paciente asociados a estancia hospitalaria prolongada en pacientes postoperados en el servicio de cirugía general del Hospital Nacional Daniel Alcides Carrión en el año 2018
Abstract
OBJETIVO: Se busca determinar los factores del paciente asociados a estancia hospitalaria prolongada en pacientes postoperados en el servicio de cirugía general del Hospital Daniel Alcides Carrión en el año 2018.
MATERIAL y MÉTODOS: El presente estudio es de tipo observacional, cuantitativo, analítico, de casos y controles realizado en el Hospital Daniel Alcides Carrión. La muestra estudiada abarcó 319 pacientes, en donde 115 pacientes presentaron estancia hospitalaria prolongada y 204 no la presentaron. Se recolectó la información mediante el uso de una base de datos del servicio de cirugía sumado a la revisión de historias clínicas de cada paciente y el llenado de una ficha de recolección de datos.
RESULTADOS: En la población estudiada se halló que el promedio de estancia hospitalaria media fue de 8 días. Se analizaron las características sociodemográficas, donde se encontró que el sexo masculino, la edad mayor de 45 años y los estados civiles sin pareja (soltero, viudo, divorciado) no se asociaron significativamente con la estancia hospitalaria prolongada (p=0,681, p=0,479, p=0,101). Sin embargo, haber llevado estudios hasta la escuela primaria resultó ser un factor de riesgo para una mayor estancia (OR=2,531). El IMC>=25 kg/m2, la presencia de comorbilidad y la programación de emergencia no se asociaron a una estancia prolongada (p=0,086, p=0,061 y p=0,073), a diferencia de las variables herida contaminada y sucia, presencia de complicaciones y cirugías abdominales previas, presentándose como factores de riesgo independientes para la estancia hospitalaria prolongada (OR=2,1, OR=9,5 y OR=5,5).
CONCLUSIONES: Haber llevado estudios hasta la escuela primaria, presentar una herida quirúrgica tipo contaminada o sucia, la presencia de complicaciones y la presencia de cirugías abdominales previas resultaron ser factores asociados a la estancia hospitalaria prolongada.
