La Amplitud de Distribución Eritrocitaria (RDW) como factor pronóstico de mortalidad en pacientes hospitalizados con insuficiencia cardiaca en el departamento de medicina interna del Hospital San José del callao, durante el periodo de Enero del 2015 a Setiembre del 2018.
Abstract
Objetivo: Analizar el RDW como factor pronóstico de mortalidad en pacientes con insuficiencia cardiaca (IC) hospitalizados en el Departamento de Medicina Interna del Hospital San José, durante el periodo de enero del 2015 a septiembre del 2018. Material y método: Se realizó un estudio observacional, analítico, retrospectivo y de cohortes. La población estuvo conformada por los pacientes con el diagnóstico de IC hospitalizados en el Departamento de Medicina Interna del Hospital San José durante enero del 2015 a septiembre del 2018, la población total estudiada es de 94 pacientes que cumplían con los criterios de inclusión y exclusión. Resultados: En el análisis multivariado se halló como factor pronóstico de mortalidad en pacientes con IC, al RDW anormal (≥14,5%) (RR= 2.00; IC=1,18 –10,55; p=0,0041), y a la clasificación ACC/AHA en las etapas C y D (RR= 9,91; IC=2,97-32,99; p=0,0002). Conclusiones: Se concluye que el RDW alterado es un factor pronóstico de mortalidad en pacientes con IC. Así mismo, se halló que la mortalidad intrahospitalaria fue de 45% de los 94 pacientes estudiados. Además, los pacientes con más de 7 días de estancia hospitalaria tienen mayor probabilidad de fallecer. También, se concluye que los pacientes que se encuentran en etapas C y D de la clasificación ACC/AHA tienen mayor riesgo de mortalidad. Por último, la mayor parte de nuestros pacientes fueron varones, mayores de 60 años y de procedencia de Lima y Callao.
