Perfil microbiológico y factores de riesgo asociados a infecciones del tracto urinario por cepas productoras de betalactamasas de espectro extendido en el Hospital San José, Callao 2010-2018
Abstract
Objetivos:
Determinar el perfil microbiológico y los factores de riesgo asociados a infecciones
del tracto urinario por cepas productoras de betalactamasas de espectro extendido
en pacientes hospitalizados en el Servicio de Medicina del Hospital San José, Callao
periodo 2010-2018.
Métodos:
Estudio del tipo observacional, retrospectivo, descriptivo, analítico de casos y
controles, teniendo como muestra a todos los pacientes hospitalizados en el
Servicio de Medicina del Hospital San José, Callao periodo 2010-2018.
Resultados:
De los 388 pacientes que conformaron la presente investigación, el 37.9% presentó
ITU BLEE y el 62.1% presentó ITU no BLEE. El sexo femenino y tener alguna
condición relacionada a la inmunosupresión fueron los factores de mayor
frecuencia. Los resultados que se obtuvieron en el análisis fueron: edad (p: 0.07,
OR: 1.028, IC: 0.682-1.550), estado nutricional (p: 0.02, OR: 1.104, IC: 0.714-1.704),
anemia (p: 0.1, OR: 1.623, IC: 1.038-2.537), ITU previa/antibiótico previo (p: 0.247,
OR: 3.226, IC: 1.988-5.236), enfermedad renal (p: 0.2, OR: 3.535, IC: 2.295-5.447),
HBP (p: 0.03, OR: 1.15, IC: 0.515-2.585), DM2 (p: 0.105, OR: 1.544, IC: 1.022-
2.334) e HTA (p: 0.08, OR: 1.439, IC: 0.952-2.176).
Conclusiones:
Se aisló a la bacteria E. Coli BLEE en 123 urocultivos (31.7%). Los pacientes que
tuvieron antecedente de ITU previa, antibiótico previo y enfermedad renal
presentaron 3 veces más riesgo de presentar ITU BLEE con respecto a los que no
presentaban estos antecedentes, sin embargo, no alcanzo significancia estadística.
